General Support:
· Assist individuals and families affected by Parkinson’s Disease cope with their emotional and physical needs. From first diagnosis, change in health status,
crisis and advanced stages of Parkinson’s. This position will be in office 5 days per week (some weekends required for special events).
First Line of Contact for People with Parkinson’s:
· Provide outreach to newly diagnosed Parkinson’s patients and families
· Track outreach to measure and report on outcomes
· Outreach can include in-person meetings, phone or zoom calls
Case Assessment of Client and family:
· Meet with clients and family at the Parkinson Association of the Rockies office. This allows an in-depth evaluation of their living environment to address
safety issues in the home. Make recommendations around safety concerns
· Educate the client at the PAR office, on the phone, and in the community
· Track data on office/telephone assessment for measuring outcomes and reporting purposes
· Provide resources in the community for residents in CO and WY related to their P.D. Including Neurologists referrals, Home Health Agencies, Senior Independent
Living, Assisted Living facilities, Skilled Nursing Home and Adult Day Programs
Support Groups:
· Help Facilitate PAR evening and Care Partner support groups
· Lead/Facilitate new support groups as necessary until volunteer leaders can be found
· Attend support groups for additional representation of PAR
Committees:
· Participate and engage in E3 and Care Partner Summit committees
o E3 is an annual conference open to People with Parkinsons, families, medical providers and the Parkinson’s community
o Care Partner Summit is specifically for Care Partners of People with Parkinson’s
Other Responsibilities:
· Maintain resource list and share new resources with team
· Quarterly education with team. Help staff understand resources and how they can help community as well as new information related to PD and treatments
· Act as community liaison and educational speaker as needed in metro area
Additional Knowledge (not a requirement):
· Knowledge of qualifications for Medicare and Medicaid
· Social Security, Social Security Disability Income (S.S.D.I.) and Supplemental Security Income (S.S.I.)
