Navigation Therapist

Navigation Therapists work with clients who are receiving Medication Only level of care. Therapists provide care management, brief therapy, outreach and engagement, treatment plans, and other applicable services to clients receiving primarily medication only services. Additionally, this position maintains an individual caseload, as well as being responsible for administrative duties of the Mostly Meds team.
This position enacts outreach and engagement strategies to special populations and provides individual support and resources to clients as needed. This position collaborates with consumers, providers, and other departments to ensure seamless care within Jefferson Center and the community.
Essential Duties:
- Provide care management, brief therapy, individual assessments, outreach and engagement, and treatment plans.
- Engages consumers in treatment process and goal setting. Identifies risk factors including lethality. Works with consumers to reach agreed upon goals.
- Responsible for the coordination and completion of needed paperwork including treatment plans, releases of information, CCAR, and other required documentation.
- Responsible for coordination of care with community providers and other departments within Jefferson Center to assure clients are receiving the appropriate level of care.
- Maintains a positive focus and excellent customer services skills while working with consumers, Jefferson Center staff, and the community. Works from a strengths-based, recovery-based perspective and takes cultural considerations into account.
Productivity, Documentation and Outcome Data:
- Documents all contacts according to Jefferson Center and Navigation program standards while meeting productivity standards as identified by the Program Director.
Other Duties:
- Exhibits enthusiasm, courtesy, adaptability, flexibility, and spirit of cooperation in the work environment.
- Maintains effective interpersonal relationships with consumers, peers, subordinates, upper management, visitors, and the general public.
- Ability to manage multiple tasks, prioritize concerns, and effectively manage time independently.
- Attends mandatory in-services, compliance with individualized training plan if required.
- Participates in supervision by coming prepared with an agenda. Reports high risk/problem cases and utilizes a problem-solving approach as well as feedback. Attends supervision at times and intervals agreed upon with supervisor.
- Submits 90% of all SRFs/Progress-to-Date forms within 3 working days (team specific).
- Completes 95% of all paperwork by the due date.
- Meets required number of consumer service hours or other measures of productivity as determined by Navigation Director.
- Works cooperatively with other community agencies, as appropriate, and in agreement with supervisor. Volunteers to work on Center committees and/or task forces.

Education, Knowledge, Skills & Experience Required:
- Master’s degree (from accredited University/College) in a mental health or related field required.
- Two or more years of experience in a mental health setting required.
- Clinical License (LPC, LCSW, LMFT) preferred.
- Bilingual (English/Spanish) preferred.
Salary Grade 51-Master’s Unlicensed Clinician $47,600 to $60,400.*
Salary Grade 52– (Licensed Clinician LCSW, LMFT, LPC) $57,000 to $72,700.*
Additional Salary Information*:
- All salaries at Jefferson Center are calculated based on years of total relevant experience.*
- The salary range above is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours/week will be prorated and adjusted to the appropriate FTE.*
**BONUS OPPORTUNITY!! Three phase bonus paid out using the following schedule. ** Based on full time (40 hours/week). Prorate if less than 40 hours/week. Must work a minimum of 20 hours/week to be eligible for bonus. Current Jefferson Center employees are not eligible for sign on bonus*
