SUMMARY: Provides case management services to enhanceresidents’ ability to manage within the Assisted Living program parameters.

■ Provides assessment, consultation, and information/referral services with Kavod’s Assisted Living residents and family membersto better enhance residents’ ability to manage assisted living.
■ Provides short-term counseling and supportive intervention for psychosocial needs, grief and loss issues, and adjustment problems.
■ Provide assistance to assisted living residents with letter writing, mail reading and making phones calls.
■ In the absence of other resources, may work with residents to complete and submit applications for SSDI/AND/HCBS/PTC.
■ Responsible for coordination, creation and completion of all resident care plans and assessments.
■ Liaison and referral for residents regardinginsurance policies, including Long Term Care Options/Medicaid program.
■ Provides coverage, intervention, documentation, and follow-up for resident emergencies.
■ Assists residents and family with finding appropriate housing when the resident no longer meets the requirements of the Assisted Living Program.
■ Provides assessment, consultation, and follow-up services for management, staff, and other involved parties when resident psychosocial issues are involved in housing management problems.
■ Coordinates with the Wellness Coordinator to monitor hospitalizations, temporary placements, and returns through resident notification, memos and incident reports.
■ Coordinates with Wellness Coordinator (H&W Program ServicesAssistant – health appointments) about resident appointments, activity attendance and sign up.
■ Runs family and care meetings.
■ Documents information regarding assisted living residents and their care in PointClick Care (PCC)
■ Leads direct service group(s), as needed.
■ Assists the Health & Wellness Department with independent program staffing needs as needed.
■ Provides periodic training to staff, or bereavement as schedule permits. Establishes and continues communication with outside partners.
■ Oversees all volunteers that work with the AL residents/staff; including Senior Companions.
■ Provides updates to family members/guardians/case workers on a monthly basis.
■ Educate, schedule and meet with residents to complete Health and Wellness Discussions.
■ Compile and enter data received from night managers’ and other non-Health & Wellness employee reports and completed Health and Wellness Discussions.
SUPERVISORY RESPONSIBILITIES: Directly supervises volunteers, Senior Companions and Russian translators. Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws. Responsibilities include interviewing, hiring, training, planning, assigning, and directing work; appraising performance; rewarding and disciplining; addressing complaints and resolving problems.
EDUCATION and/or EXPERIENCE: Bachelor’s degree in Human Behavioral Science which includes 30 semester hours of intervention techniques, diagnostic
measures, or therapeutic techniques, such as social work, psychology, sociology, guidance and counseling, and gerontology, plus two years’ experience working with seniors.
