Supported Employment/ Pre-Vocational Specialist/Case Manager

Responsibilities:
- Business Operations: fulfills assigned responsibilities related to the Entrepreneurial Operating System.
- Fosters trusting, consistent and collaborative relationships with members, guiding them towards competitive employment in inclusive community settings.
- Delivers personalized, on-site coaching of essential job skills to individuals with disabilities, facilitating their success in securing and maintaining fulfilling employment.
- Collaborates with members to create customized employment and education plans, incorporating input from member teams and family members.
- Conducts regular assessments of members’ vocational abilities and aspirations, adjusting plans as needed. Tailors job development and search activities to match individual strengths and goals.
- Assists members in obtaining information about their benefits (e.g., SSI, Medicaid, etc.) and how they will be affected by employment.
- Appropriately provides education and support to family members, caregivers, and host home providers.
- Cultivates relationships with prospective employers through outreach. Serves as a dedicated point of contact for employers.
- Follows the principles and procedures of IPS supported employment.
- Maintains minimum employer contacts and member job start requirements.
- Creates monthly progress reports to submit to DVR that include services provided and challenges members may face.
- Completes daily notes of individual’s progress and challenges, timesheet and goal tracking in Therap.
- Works with the program Director to update and maintain program participant records.
- Transports members to and from work/interviews, if required.
- Provides medication administration to individuals served, when required.
Qualifications:
- Bachelor’s degree in a Human Services or equivalent preferred.
- Certified Employment Support Professional (CESP) certification, or ability to obtain within 30 days of hire required.
- Association of Community Rehabilitation Educators (ACRE) preferred.
- Qualified Medication Administration Personnel Certification (QMAP), or ability to obtain within 30 days of hire.
- CPR and First Aid Certifications, or ability to obtain within 30 days of hire.
- Safety Care certification, or ability to obtain within 90 days of hire.
- Demonstrated professionalism, empathy and respect when interacting with diverse individuals and navigating complex situations.
- Ability to maintain an unwavering positive attitude and resilience in the face of challenges.
- Strong ethical standards, including honesty, self-discipline, and personal accountability.
- Effective and respectful communication in both verbal and written formats, adapting to individual and group settings.
- Proficient computer skills and digital literacy.
- Reliable, supportive, and collaborative team member.
- Ability to work an open and flexible schedule to meet assigned member and program needs, including some holidays, nights and weekends.
- Ability to work with sensitivity to the cultural and/or socioeconomic diversity of the agency’s clients and staff.
- Ability to maintain a high level of confidentiality and trust.
- Must pass a background check, drug screening, and physical exam.
- Valid driver’s license and insurance with the ability pass a multi-point safety inspection through an ASE certified mechanic and the ability to travel on behalf of the organization.
- Must be at least 21 years of age.
