Outpatient/Substance Use Case Manager
Nature of Position:
The is an Outpatient Case Manager administrative position. The Outpatient Case Manager is responsible for counseling and conducting needs assessment.
Conducts thorough Case Management Needs Assessment and consistently re-screens for additional needs of client participating in Substance Use Disorder Treatment. Connects clients with community support and resources before, during, and after treatment. Supports clients with additional life skills resources and training where appropriate. Assessing and providing group and individual counseling for clients in Substance Use Disorder Treatment as directed, discretion and supervised by Clinical Substance Use Program Supervisor.
Job Duties:
- Knowledge of Substance Use Disorders, Mental Health Disorders including assessment and treatment Modalities.
- Provide professional case management services that facilitate coordination, communication, and collaboration with clients, providers, ancillary services, and others in order to achieve goals that maximize positive client outcomes based upon individual assessments of clients’ needs.
- Provides individual and group therapy, as directed by Clinical Supervisor and/or Director
- Produces and ensures high quality and accurate case management documentation within 24 hours of service being provided.
- Facilitates consistent communication with family members, significant others, and fiduciaries.
- Maintains a working knowledge of and accountability to the policies and procedures of the facility. .
Physical requirements include the following:
- Frequently stays stationary for prolonged periods of time.
- Frequently moves within and between facilities.
- Frequently exposed to stressful or emergent situations.
- Frequently requires utilization of stairs
- Frequently operates a computer and other office equipment to record health data.
- Must be able to talk and hear as position requires constant communicate with clients and others.
- Occasionally moves equipment and/or materials up to 20 pounds.
- Occasionally drives between facilities or for other assigned duties.
Qualifications:
- High School Diploma or Equivalent/ Bachelors degree or higher in Substance Use Disorder/Addiction and/or related counseling subjects (i.e. Social Work, Mental Health Counseling. Marriage & Family, or Psychology)-from a regionally accredited institution of higher learning.
- Current Colorado Credential as Certified Addiction Technician (CAT), Certified Addiction Specialist (CAS) or Licensed Addiction Counselor (LAC) in good standing or be eligible to obtain Certification or Licensure.
- 1 or more years providing substance use disorder treatment and/or mental health case management.
- Valid and current Colorado driver’s license and access to transportation during work hours
- Ability to pass a background check and drug screen.
- 1 or more years office management experience in a professional setting
- Customer service, computer and organizational skills.
- Minimum two years freedom from chemical use problems
- Access to reliable transportation.
