Case Manager/Community Affairs Representative

Duties/ Responsibilities:

· Provide intensive case management and supportive services to homeless families residing in emergency and transitional housing (1/2 time).

· Keep accurate and timely case recording and client data.

· Provide appropriate community referrals to clients and callers seeking services.

· Conduct comprehensive intake assessments with potential clients.

· Conduct regular home visits.

· Ensure confidentiality and security of client information.

· Represent and advocate for the client in their interaction with private and governmental organizations.

· Represent Family HomeStead to the community. to other homeless service providers, and to governmental and non-profit organizations involved in providing services to the homeless population in Metro-Denver (1/2 time)

· Other duties as assigned


· Bachelor’s degree in social work, human services, or a closely related field required (Master’s degree preferred).

· A Minimum of 5 years of professional experience working with homeless and/or extremely low-income families or individuals.

· Must have a valid driver’s license and a reliable vehicle.

Salary Range:  $40,000 (meets minimum requirements) to $60,000 (significantly exceeds maximum requirements)
