Mission: EChO champions people on their unique path from survival to success. Our services are designed to assist community members who are facing personal or economic adversity by providing a range of wrap-around services tailored to the specific needs of individuals and families. EChO’s programs include the Food Pantry, Client Services, Education Center, Evergreen Shelter Program and the EChO Resale Shop. EChO serves a diverse population primarily consisting of residents of the unincorporated mountain areas of Jefferson, Clear Creek, Gilpin, and Park counties.

We are currently seeking a Case Manager to provide comprehensive wrap-around case management services to address the immediate and long-term needs of EChO clients.

Essential Responsibilities:

· Provide intensive case management to caseload, including shelter residents

· Conduct in-depth assessment(s) of caseload to identify needs, strengths, and barriers to stability

· Response and evaluation of crisis situation(s), provide support, assessment, and connection to
appropriate resources

· Advocate for clients in complex systems such as behavioral and physical healthcare, criminal justice system, housing resources, etc.

· Establish and maintain collaborative relationships with clients, community partners, community resources, and other EChO staff members/departments

· Coordination and completion of paperwork including housing application/voucher applications, shelter and HMIS registrations, and benefit applications such as Medicaid, SNAP, OAP, TANF, etc.

· Respond to caseload with trauma informed lens and perform essential employment responsibilities with a person-centered, strengths-based approach

Other Responsibilities:

· Ability to multi-task, prioritize concerns, and effectively manage time independently

· Appropriate, timely, and professional documentation of client notes

· Effective and timely response to client/consumer needs via phone calls, texts, emails, in-person, etc.

· Participate in weekly supervision, come prepared with agenda and topics for discussion

· Participate in ongoing professional development

· Participate in off-site EChO events, in rare instances, a weekend or evening may be required

· Participate in weekly team meetings/monthly staff meetings

· Other duties as assigned by Director of Program Services

Education, Knowledge, and Skills Required:

· Master of Social Work degree preferred

· 2-3 years case management experience preferred

· Experience in social services or non-profit agency preferred

· Strong understanding of issues related to homelessness, mental health, and substance use concerns

· Strong communication, organizational, and computer skills

· Ability to work collaboratively and independently

The Case Manager is a full-time, on-site, non-exempt position, working 40 hours
per week, with a range of $24-$26 per hour.
