Case Manager – Coverage + Bilingual

Developmental Pathways (DP)has an opportunity for a Case Manager – Coveragein our Systems Navigation department who will manage caseloads in coverage to ensure support is provided, aligning with rules and regulations. This role will also complete coverage tasks for individuals in coverage including required documentation related to screening and referral, eligibility determination and assessments, support plan development, PMPM activities, monitorings, and other tasks as needed. In addition, this role will provide support with coverage management, research, and assignments and will work with Program Manager to determine needs amongst coverage. This role will act as a systems navigator, assessing an individual’s overall needs and assisting with exploring programs, long-term services and supports, and other community resources to meet needs.

This is a 40-hour week, hybrid position, working remotely and coming into our headquarters office in Aurora, CO and/or off-site locations, as needed.

Starting Salary & Benefits
$22.24 – $24.46 hourly+ an additional 4% Bilingual pay premium for hours worked
