About the Role
In this position, you will provide case management services to people with disabilities and older adults who have a trust with CFPD. You will help trust beneficiaries access their funds while keeping them in compliance with programs like SSI and Medicaid. This includes assisting your beneficiaries in planning and budgeting for their needs, collaborating with their family and dedicated team members, advocating for your beneficiaries, and facilitating the use of their funds.
A Day in the Life at CFPD
During any given day as a trust case manager at CFPD, you might…
- Have email and phone conversations with beneficiaries, family members, and service agencies to keep them updated and answer questions about how the trust works, the trust balance, or to provide updates about the status of a purchase or request.
- Write detailed requests on behalf of the beneficiary for the trust to pay for expenses like a phone bill, a dental exam, a laptop, or an upcoming summer trip, and pay their recurring monthly bills online and by check.
- Update an ongoing record of time spent working for each beneficiary, organize the week’s payment backups, and attend a department meeting where you’ll receive support from your co-workers and discuss the latest changes in regulations and procedures.
- Reach out to a guardian about a beneficiary’s pressing need, update a beneficiary’s budget based on their remaining funds, or refer a beneficiary to a program or organization that will benefit them, even helping them sign up and fill out the paperwork.
- Venture out into the community to meet with a new beneficiary in their home to get to know them, assess their needs, and help them make a detailed spending plan for the lifetime of the trust. You might drop off a new phone to a beneficiary’s home, or even meet them at a nearby store to help them buy some winter clothes.
What You Will Bring Along:
- Demonstrated knowledge of public/government benefits, including SSDI, SSI, and Medicaid.
- Demonstrated understanding of special needs trusts.
- Excellent written and oral communication skills.
- 1-3 years of experience working as a Case Manager within the human services field.
- Clean background check & satisfactory credit report.
- Driver’s license, a working automobile, and a willingness to drive within the Denver Metro and surrounding areas and occasionally transport beneficiaries.
- Ability to speak clearly and hear well due to heavy telephone contact.
- Bilingual candidates are highly encouraged to apply.
