Community Nurse – RN
Summary
The Colorado Coalition for the Homeless seeks a Community Nurse who works collaboratively within an Integrated Health Care for the Homeless and supportive housing teams. This position is responsible for performing both independent and delegated duties in the provision of providing comprehensive health care to assigned patients within the Colorado Coalition for the Homeless (CCH).
The Community Nurse primarily provides services in the community within the framework of the nursing scope, professional nursing ethics, and state and federal laws and regulations. This is a field-based position with most of the services provided to patients in settings outside primary health center sites. The nurse case manager collaborates with other integrated health and supportive housing service team members to deliver care to clients living on the streets and in supportive housing communities. This position is located at 2100 N. Broadway Denver CO and reports to the Nurse Manager.
NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date. The credentialing and privileging timeline is dependent on the selected candidate’s submission of documents necessary for clearance. Please note that a start date will be scheduled once credentialing and privileging is complete.
Essential Job Functions
- Provides skilled nursing care according to accepted community standards of quality integrated healthcare. This includes the ability to perform quick and accurate medical and psychiatric assessments, treatment, and follow up according to the nursing process.
- Assists with coordination of health care provided to consumers in the community including linkage to higher levels of care.
- Is responsible for providing nursing care management activities to coordinate integrated patient care, decrease harm, and enhance wellness in population.
- Empowers patients by providing appropriate health education and information that is tailored to both the individual’s learning style, education and/or reading level, and is sensitive to their health beliefs to promote recovery, wellness, and optimal health.
- Provides medication management and medication administration under the supervision of the designated provider. Administers medications, including intramuscular injections in a safe, evidence-base manner following written protocols or direct provider order.
- Assesses and monitors consumers’ ongoing psychiatric, medical, and social needs. Collaborates with internal and external providers to coordinate psychiatric treatment with medical treatment; acts as a psychosocial educational resource by assisting consumers in learning about their mental illness, physical health conditions, medications, and how to protect their overall health.
- Coordinates patient’s transitions of care.
- Performs treatments and laboratory procedures independently and/or as requested by the health care provider.
- Provides patient-centered care within an integrated, trauma-informed, harm-reduction model.
- Collaborates and coordinates care with other members of the CCH integrated multi-disciplinary health care teams including primary care, behavioral health, psychiatric, and dental providers, other RNs and MAs, case managers, peer specialists and outreach associates, and other clinical and administrative staff as required.
- Coordinates care with internal programs including but not limited to Health Outreach Program, Recuperative care, and Medication Assisted Treatment Program.
- Provides treatment services such as case management, crisis services, initial intake program screenings, individual medication assisted treatment (MAT) counseling, and administering scales to assess withdrawal.
- Provides transitional, chronic, and complex care management.

- Rigidly adheres to Universal Precautions per CDC guidelines and OSHA regulations.
- Rigidly adheres to HIPAA regulations per standards developed by the Department of Health and Human Services.
- Participates in team meetings, in-services, and supervisory sessions as required.
- Must be accessible by cell phone during business hours.
- Utilizes the Electronic Health Record (EHR) as directed.
- Documents appropriately in Safetyzone for internal reporting of client or staff events.
- Completes required onboarding, orientation, annual competencies, and assigned continuing education.
- Participates in team-based or interdisciplinary quality improvement initiatives.
- Attends required disciplinary, inter-disciplinary, suite, quality improvement, and all-staff meetings.

- Performs other duties as assigned by management.
Qualifications Summary
- AD, BSN, or MSN in nursing from an accredited college or university.
- Current licensure from the State of Colorado to practice as a Registered Nurse.
- Certification in Basic Life Support.
- Successful completion of the privileging and credentialing process for RNs at the Coalition.
- Preferred experience in psychiatric, care coordination, hospice, ambulatory, public or community nursing.
