Purpose of Position: The Quality Initiatives Specialist provides training and technical assistance to Colorado’s Community Health Centers (CHCs) to enhance their clinical and quality improvement work. The Quality Initiatives Specialist has the opportunity to support CHCs in a wide variety of areas; current primary focus areas for this position include cancer screening, chronic disease prevention and management, and team-based care.
The ideal candidate for this position has experience in health care quality improvement, is interested in working with teams at Community Health Centers to accomplish their goals in a primarily virtual setting and is adept at making connections across multiple areas of work to facilitate and participate in peer learning. This candidate will be mission driven and aligned with CCHN’s values and demonstrate flexibility and curiosity in how they approach their work.
Essential Duties/Responsibilities:
Guides the quality improvement of Community Health Centers (CHCs).
Facilitates and coordinates CHC training and technical assistance including general practice facilitation, workflow development, and coaching multidisciplinary teams.
Collaborates with the CHCs to facilitate data collection, reviews data to inform report development, presents data to CHCs and partners, and uses data to support CHC improvement efforts.
Monitors deliverables for projects, including data collection and CHC reporting.
Collects and shares best practices and successes among CHCs across multiple areas of CCHN’s work.
Collaborates with key partners and funders to present project updates and prepare progress reports.
Develops content and resources for CHCs and partner audiences, including but not limited to fact sheets, resource lists, trainings, and toolkits.
Attends and represents interests of CCHN and CHCs at relevant task forces and partner meetings.
Plans and facilitates internal and external meetings, trainings, webinars, conference calls, and workgroups. While meetings may be in-person or virtual, the majority are currently virtual.
Prepares and presents written and verbal updates for CCHN peer networks as requested.
Travels to Colorado CHCs to provide program-specific technical assistance approximately 10% of the time, and in accordance with CCHN’s Hybrid/Remote Work Policy.
Other Duties/Responsibilities:
Fosters external partnerships with key stakeholders (including, but not limited to, state agencies, coalitions, funders, etc.) by attending regular state- and region-wide meetings, conferences, and networking events , as assigned, to represent interests of CCHN and CHCs.
Works in partnership with Quality Initiatives Division staff to facilitate, coordinate, and take notes for peer learning groups of CHC staff.
Reports corporate compliance concerns to CCHN’s Corporate Compliance Officer; any CCHN employee who reports a compliance concern in good faith is protected by law from retaliation.
Assume other related responsibilities as assigned.

Knowledge, Skill, and Ability:
Ability to facilitate group discussions that challenge participants to consider various approaches/solutions.
Knowledge of, or ability to learn about Community Health Centers, safety net clinics, or primary care settings, including clinical practice models and best practices.
Experience or familiarity with topics of cancer screening, chronic disease, team-based care, patient-centered medical home, and/or quality improvement preferred.
Proficiency in Microsoft Office software, including Word, Excel, and PowerPoint.
Proficiency in using virtual meeting platforms such as Microsoft Teams and Zoom.
Fluency in written and spoken English
Proficiency in written and oral communication skills.
Ability to lead, plan, and manage change among teams.
Ability to analyze, organize, and evaluate data.
Ability to work with customer/client groups and/or experience in membership organizations.
Training/Education:
Bachelor’s Degree in a related field or experience in a relevant field, such as primary care or Community Health Center (CHC) setting. See also desired experience, below.
Experience:
At least two years of professional office-based experience, with at least some of the following areas preferred:
Community Health Center or primary care
Quality and process improvement
Facilitating peer learning, practice coaching, and/or health care quality improvement
Meeting development and facilitation
Working Environment and Physical Activities:
The person holding this position must reside in Colorado.
CCHN currently follows a hybrid work structure where employees will work remotely and in CCHN’s office at 600 Grant Street, Denver, CO. The policy is subject to change, but currently allows for approximately 80% remote work. CCHN currently has one all-staff in-office day per month with additional in-office time expected based on team, organization, and project needs.
Ability to travel. This position typically requires several hours of travel to meetings in the Denver or Colorado statewide area each month and will require several in-state overnight trips per year. Out-of-state overnight trips may occur, approximately once per year or less.
