Position Summary:
The Case Manager at Adam’s Purpose plays a vital role in providing grief navigation, case management, and resource coordination for individuals and families impacted by loss. This role supports grieving participants by conducting intake assessments, providing direct resource referrals, coordinating care, and ensuring clients receive culturally responsive support tailored to their needs.
This position is part-time (15-25 hours per week) with the potential to transition into a full-time role as funding allows. The role is hybrid, requiring remote work and in-person engagement at events, support groups, and partner sites in the Denver area.
Key Responsibilities
Client Support & Case Management
– Conduct intake assessments and develop individualized support plans for grieving individuals and families.- Provide case management services, including identifying and connecting clients to internal programs, mental health providers, grief support services, social services, and community resources.- Serve as a consistent point of contact for program participants, ensuring follow-up, progress tracking, and emotional support.
Program & Participant Engagement
– Work closely with AP Team, Peer Support Facilitators, Healing Circle leaders, and therapists to ensure participants receive well-rounded support.- Assist in the coordination of support groups, healing circles, and grief education programs by managing participant engagement and logistics.
– Serve as a point of contact for participants, addressing questions, providing guidance, and ensuring a welcoming, supportive environment.
-Manage participant sign-ups, attendance tracking, and follow-ups.
-Ensure smooth facilitation by coordinating supplies, space, and scheduling for Volunteer Liaison & Support
Assist in the coordination and implementation of monthly Healing Circles, ensuring seamless organization and participant engagement.
Monitor program impact, collect feedback, and help refine program delivery based on participant need
Resource Navigation & Community Outreach
– Maintain an updated directory of mental health providers, grief counselors, social services, and culturally responsive resources for referrals.- Assist families in navigating financial assistance, childcare, transportation, and other critical needs related to their mental health and grief journey.
– Participate in Community outreach to support with recruitment or sharing about AP Programs.
Documentation, Reporting & Impact Measurement
– Maintain accurate and detailed case notes, referral tracking, and client progress reports.- Collect and analyze data on participant engagement, outcomes, and program effectiveness.- Ensure compliance with confidentiality and ethical guidelines when handling participant information.
Education & Experience Requirements
– Bachelor’s degree in Social Work, Psychology, Counseling, Public Health, or a related field (preferred but not required with relevant experience).- 1-3 years of experience in case management, mental health support, grief counseling, social services, or community-based work.- Strong understanding of grief, trauma, and culturally responsive care in Black and BIPOC communities.

-Tech-savvy with experience using Microsoft Office, Google Workspace, and case management software for documentation and reporting.
-Flexibility and adaptability in a hybrid work environment, with some evening and weekend availability required for events or support sessions.
Schedule & Compensation
– Part-time (15-25 hours per week), with potential to become full-time based on funding and program needs.- Hybrid work model – mix of remote work and in-person engagement in Denver.- Bi-monthly pay 5th and 20th of the month – Opportunities for professional development and potential career growth within Adam’s Purpose
